The objective of this assignment is to demonstrate the ability to organize and describe case material using cognitive and behavioral theories and develop an assessment and treatment plan

Assignment:

Identify a case, or some other helping role to use for the paper. A brief biopsychosocial summary of the case should be provided (

2-3 pages

) that includes the presenting problem as if it were being placed in a medical chart (see suggested format for the biopsychosocial summary below). The student should write up a case formulation in a narrative form (NOT handed in as a completed chart).

TASKS:

  • Explain the target issue using behavioral theory, including the terms and concepts of the theory.
  • Explain the target issue using a cognitive theory lens, clearly identifying the concepts and terms used in CBT (see below for details).
  • Using the combined CBT approach (behavioral and cognitive), describe the case in a NON-JARGON/CLIENT FRIENDLY manner (can be written as a summary to a caregiver or colleague or as if you were speaking directly to the client).

(The connection between the thoughts, feelings, and behaviors should be clear as you explain why the person is feeling, thinking, and behaving in the way that they present to the worker).

Described the client addressed in each area described in the assessment outline (

Pertinent History:

Provide a brief description of the client, including cultural background, living situation, the family system, health and mental health issues, substance use history, occupational and educational status, and other pertinent information related to your assessment.

    Reason for referral and some history of problem

  • Assessment Procedures

    : Describe instruments (e.g., Beck Depression Inventory BDI II, clinical interview) use may have used. You can fill in what you may have done or which assessments you would use if you are not working with an actual client.

    Clinical issues and Readiness for Change (consider principles of MI and stages of change)


2. Case Formulation and Recommendations for Intervention

This is the focus and “heart” of this assignment (see format below)


  • Target Issue

    : In a separate paragraph, clearly identify the target issue (i.e., a behavior, a feeling, a reaction) including the frequency, duration, intensity, and any other descriptors that help “paint a picture” of the main issue

  • CBT narrative conceptualization:

    In a narrative format, use the cognitive conceptualization and case formulation chapters in Tolin as a guide (Chapters 5 &6) and Chapters 21 & 22, Ana and Elizabeth’s CBT assessments. Link the formative and maintaining influences on the current issue. It should be an explanation using cognitive behavioral theory and NOT a description or re-summary of the case material.

  • CBT diagram conceptualization:

    Use the Meaty Conceptualization from Tolin’s book to describe a specific episode of the client’s presenting problem. (

  • Treatment Plan:

    Describe the treatment plan and identify an intervention for at least ONE target. Make sure you show your knowledge of terms such as cognitive restructuring, automatic thoughts, and schema in your narrative and utilize the cognitive cycle (e.g., from Meichenbaum) to inform what the intervention might entail for the client. Explain the rationale behind the chosen target and the choice of intervention. Reference why the interventions is appropriate and feasible for the client’s problem as well as individual issues of that client, with attention to culture, spirituality, gender identity, class, sexual orientation, race, and ethnicity (

    1-1.5 pages

    ).

It should be comprehensive and parsimonious and provides an accurate clinical summation of the relevant issues that are currently influencing the situation of the client using the CBT framework (connect all three points of the cognitive triad here – integrate the CT and BT sections from above). You may include a thought and feelings record or ABC charts. (
